Student Health Records
The SNAP Health Center student Health Record is the epicenter of student medical data, past and present.
Each record provides quick access to all the same information as the traditional health card, with the addition of medication
and visit information. Data is categorized within each record into separate sections or tabs: Student, Home, Contacts, Flow Charts, Immunizations, Absences, Alerts/Conditions, Screenings, Medications, History/Activities, Billing and IHP’s. Virtually all the health data that you could ever need for a student is right at your fingertips.
Student Health Records are versatile. On top of keeping student health data organized and accurate, these records are also designed
to help you save time throughout the day. This is accomplished by linking most of the functions within SNAP Health Center through
each individual record. The following descriptions outline just a few of the highlights of the Health Record.
Student & School Information
Displays student demographic data as well as individual school related information, such as teacher, grade, school ID, etc. Special notes and permission to treat/transports are immediately visible.
Home & Demographic Data
Displays additional demographic data, address
information as well as parental/caregiver and sibling data. Insurance/Medicaid data is also accessible.
Personal Contacts
Stores personal contact data with extra space for comments.
The Contacts Library makes it effortless to apply common contacts, such as local doctors, dentists, etc.
Absence History
Documents the reason and date of student absences. This allows
you to identify and track students who are absent for a certain number of days, specific reasons or even particular days of the week.
Health Alerts/Conditions
Stores confidential and non-confidential health alerts & medical conditions. For security purposes, only those with the appropriate user security rights can view confidential alerts. Three different priority levels allow you to set the importance of an alert. Notification immediately displays when a student with an alert is accessed. Also keeps a running list of past diseases, current conditions or health related incidents. This can help you to spot and track potential school outbreaks and infestations.
Health History/Activities
